NOTECARD ORDER FORM

There's another order form for original artwork—"ARTWORK ORDER FORM".

If you'd like to place an order:

• Please fill out this form with the appropriate quantities and total.
  • I am currently using UPS for shipping, and they will not deliver to a P.O. box. Please give me a valid street address for shipping.
• E-mail this order to me and tell me whether you'd like to pay with check, money order or credit card.
• I will check it over, total your order and respond via e-mail.
  • If you are paying by check or money order, I will e-mail my address.
  • If you are paying by credit card I will request your phone number so that I can call you for your card number (it's currently the most secure way).
• I will ship your order as soon as I receive payment and the check or credit card transaction has been approved.
• Once the transaction has been completed, I destroy all record of your credit card number, and I will never, ever sell your phone number or shipping or e-mail address to anyone.

If you live in the state of Pennsylvania, I need to charge 7% sales tax. Sorry. I don't like it either.
